The CMS Design System is maintained by the Centers for Medicare & Medicaid Services (CMS) to build Section 508 compliant, accessible interfaces across Medicare, Medicaid, and Healthcare.gov. CMS built its own React components and CSS utilities on top of the U.S. Web Design System (USWDS), plus a grid framework for federal healthcare products.
CMS.gov and Healthcare.gov each run their own theme on top of a shared core component library. The project is open source, maintained on GitHub, and released in regular versioned updates.
